ST. JACOB, Ill. — CSX Transportation (CSX) investigators reported that the June 27 derailment of a freight train at St. Jacob, Ill., was caused by a coupler retainer plate that failed.

The Jacksonville, Fla., transport concern filed the report with the Federal Railroad Administration, and said it is working to compensate affected individuals for damages that resulted from the release of ammonium nitrate and hydrogen peroxide.

CSX has completed environmental remediation that replaced affected soils, and the company intends to pay other expenses, including first-responder services in Madison County, Ill.

A two-person crew was operating the 131-car train and was uninjured.
