WASHINGTON, D.C. — Failures at every level, from both Conrail and first responders, contributed to a dangerous situation during the Paulsboro train derailment and chemical spill in 2012, but the probable cause of the accident was a train being allowed to proceed past a red signal that morning, the National Transportation Safety Board determined Tuesday.
