CLEVELAND, October 4 – It has come to the attention of the International Brotherhood of Teamsters that many members affected by Hurricanes Katrina and Rita are having difficulty obtaining the proper forms needed to apply for assistance.
In order to assist members impacted by the storms, attached are the Intact Forms for American Red Cross’ Financial Assistance; a FEMA application/registration for Disaster Assistance form; and the Teamsters Disaster Relief fund application. For a PDF of these forms, go to:

Red Cross forms should be returned to the closest Red Cross Relief Center or returned to the IBT and they will be delivered to the Red Cross. The FEMA application should also be filled out and either returned to the nearest FEMA relief center or to the IBT to be delivered to FEMA. IBT forms should be returned to the Fund office.
The IBT is currently assisting over 4,000 members. The Teamsters Disaster Relief Fund is also asking for donations in order to assist these members. The fund has raised $400,000 – far short of the $2 million necessary just to provide basic emergency assistance to affected members.
All donations made to the Teamsters Disaster Relief Fund go directly to members affected by the hurricanes. To make a donation, please mail your checks to the Teamsters Disaster Relief Fund, 25 Louisiana Ave. NW, Washington, D.C. 20001.
