CLEVELAND, May 5 — The Teamsters Disaster Relief Fund is accepting applications for assistance from members in six different states after severe storms, flooding and high winds damaged homes and property over the past several days.

Assistance is available to members and households in storm ravaged counties in Alabama, Georgia, Mississippi, North Carolina, Oklahoma and Tennessee.

Available below are Teamster “Disaster Alert Notices” and “Request for Help” forms for each state listed above. If you have experienced a loss due to extreme weather, please download and fill out the forms below, or call the Teamsters Disaster Relief Fund office at (202) 624-8971.

ALABAMA

The Federal Emergency Management Agency (FEMA) has declared a major disaster for the state of Alabama caused by severe storms, tornadoes and straight-line winds, which occurred on April 28, 2011. Members who live in the following counties are eligible to apply for assistance:

Autauga, Bibb, Blount, Calhoun, Cherokee, Choctaw, Cullman, DeKalb, Elmore, Etowah, Fayette, Franklin, Greene, Hale, Jackson, Jefferson, Lawrence, Limestone, Madison, Marengo, Marion, Marshall, Saint Clair, Sumter, Tallapoosa, Tuscaloosa, Walker and Washington Counties.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 20, 2011.

GEORGIA

The Teamsters disaster relief fund recently received a “disaster alert notice” for the state of Georgia regarding severe storms, tornadoes and straight-line winds that caused major damage on April 29, 2011. Members who live in the following counties are eligible to apply for assistance:

Bartow, Catoosa, Coweta, Dade, Floyd, Greene, Lamar, Meriwether, Monroe, Morgan, Pickens, Polk, Rabun, Spalding, Troup and Walker Counties.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 20, 2011.

MISSISSIPPI

Teamster disaster relief assistance is also available in Mississippi regarding severe storms and flooding that caused major damage. Members who live in the following counties are eligible to apply for assistance:

Chickasaw, Choctaw, Clarke, Greene, Hinds, Jasper, Kemper, Lafayette, Monroe, Neshoba and Webster Counties.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 20, 2011.

NORTH CAROLINA

Disaster relief assistance is available in North Carolina following severe storms and flooding that caused major damage on April 16, 2011. Members who live in the following counties are eligible to apply for assistance:

Beertie, Claden, Cumberland, Halifax, Harnett, Johnston, Onslow, Wake and Wilson Counties.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 6, 2011.

OKLAHOMA

Disaster relief assistance is available in Oklahoma after severe storms, tornadoes and straight-line winds caused major damage on April 14, 2011. Members who live in Atoka County are eligible to apply for assistance.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 13, 2011.

TENNESSEE

FEMA declared a major disaster in the state of Tennessee due to damage caused by severe storms, tornadoes and straight-line winds that occurred on May 1, 2011. Members who live in the following counties are eligible to apply for assistance:

Bradley, Greene, Hamilton and Washington Counties.

Members who wish to apply for disaster relief funds should print and complete this “Request for Help” form.

Forms (including all requested documentation) are due by Monday, June 20, 2011.
